Salem State University Institutional Review

UNSIGNED INFORMED CONSENT

This needs assessment is part of a research project for the MSW program at Salem State. I understand that this project is studying the demographics, lived experiences, and needs of transgender residents in Essex, Middlesex, and Suffolk counties.

I understand that my participation is completely voluntary and that I may stop completion of the survey at any time. I understand that I may not receive any direct benefit from my participation in this study. I also understand that some people may find it troubling to participate in some or all of the study activities and that I may decline to participate in any portions which make me feel uncomfortable.

I understand that the risks are potential emotional and/or psychological triggering. Potential benefits are that it may be used to inform agencies, resource centers, and community leaders in regards to feedback and suggestions for improvement and needed services. I understand that all answers will remain completely anonymous. My part of the study involves approximately 30-45 minutes to fill out the needs assessment.

The information provided to the researchers will be kept confidential with the exception of information which must be reported under Massachusetts’s law, such as a disclosure of risk to myself and others. An analysis of the results and an explanation of the study will be available in the Salem State University Masters of Social Work Program Academic Building in May 2011.
